Nature of the work

Most doctors within the speciality work in obstetrics and gynaecology (O&G), many have a significant interest in a particular field like high-risk obstetrics, minimal access surgery or fertility care. It is a varied speciality, often dealing with healthy women, where uncertain challenges can present themselves on an everyday basis. Work includes a significant number of hands-on procedures, both in gynaecology and obstetrics.

In gynaecology, patients differ from those who have chronic disorders but are not life-threatening (however, interfere naturally with quality of life), to those where the first indication of a gynaecological problem is an acute emergency presentation. Gynaecology is concerned mainly with the health and well-being of the female reproductive organs and the ability to reproduce. It also includes endocrinology, pelvic malignancy and female urology.

In obstetrics, most women, even though pregnant, are otherwise healthy and fit. Still, others will have acute or chronic medical conditions that can lead to complications during pregnancy and are under the special care of an obstetrician. Many new procedures and techniques have been developed during the past 30 years. And have led to the transformation in the health of women and babies. Advancements in ultrasound have made care of the baby in the womb an essential part of the speciality.

Common procedures

Obstetrics

In obstetrics, midwives take care of most mothers who are at low risk and perform uncomplicated deliveries.

An obstetrician undertakes about 35% of births, usually for more complicated cases or if the baby becomes distressed during labour. The work includes:

  • using instruments to support delivery – that includes forceps or a ventouse (vacuum-assisted delivery)
  • carrying out caesarean sections, either as a planned or emergency procedure

Gynaecology

Within gynaecology procedures include:

  • carrying out surgical interventions following miscarriage
  • treating abnormal bleeding and polyps
  • major surgery for gynaecological cancers
  • minimal access surgery for problems including endometriosis

Keyhole or minimal access surgery is now commonplace and has many advantages.

Assisted reproduction (fertility treatment) has helped many thousands of women have a family, which would not have been possible in the last generation.
